Ngón Tay Cò Súng (Trigger Finger)
Nguyên nhân gây ra ngón tay cò súng:
Ngón tay cò súng còn gọi là ngón tay lò xo hay ngón tay bật, là tình trạng viêm gân, bao gân gấp ngón tay gây nên phì đại và quá sản sụn sợi ở bề mặt tiếp xúc của gân và bao gân (Thường tại vị trí của ròng rọc A1), làm cho bao gân dày lên gây chít hẹp đường hầm của gân. Sự chít hẹp này làm gân di chuyển qua ròng rọc sẽ khó khăn.
Nếu tình trạng viêm chít hẹp nặng có thể khiến gân gấp bị kẹt lại làm cho ngón tay không thể cử động được, do lực duỗi ngón tay thường không thắng được phần nghẽn này nên ngón tay thường ở tư thế gấp giống như cò sung. Nếu cố duỗi hoặc duỗi thụ động ngón tay sẽ thấy tiếng “bật”khi cục xơ vượt qua chỗ hẹp và ngón tay duỗi ra như kiểu ngón tay lò xo.
Ngón tay cò súng hay gặp ở đối tượng nào:
- Thường gặp ở người trên 45 tuổi, nữ nhiều hơn nam
- Người bị đái tháo đường
- Có tiền sử viêm khớp dạng thấp
- Tiền sử vi chấn thương bàn ngón tay lặp đi lặp lại
- Tiền mãn kinh hoặc mãn kinh
Triệu chứng lâm sàng:
– Đau ngón tay tại vị trí viêm (Thường tại vị trí của ròng rọc A1)
– Khó cử động ngón tay, nặng nề hơn vào buổi sáng và cải thiện vào ban ngày
– Ngón tay có thể bị kẹt ở tư thế gấp, khi cố duỗi ngón tay có thể thấy tiếng “bật”
– Sờ dọc gân gấp có thể thấy cục xơ nhỏ dọc trên gân gấp ngón tay (Thường tại vị trí của ròng rọc A1), cục xơ di động khi gấp duỗi ngón tay.
Điều trị:
Bảo tồn trong giai đoạn sớm hoặc triệu chứng lâm sàng ở mức độ nhẹ:
- Nẹp duỗi ngón vào buổi tối để tránh đau do co ngón tay khi ngủ
- Hạn chế vận động ngón tay
- Thuốc chống viêm, giảm đau
- Tiêm corticoid tại chỗ
Phẫu thuật khi điều trị bảo tồn thất bại:
- Cắt ròng rọc A1 đề giải phóng chít hẹp bao gân gấp
- Sau mổ bệnh nhân có thể vận động sớm để tránh dính gân
